I'm so glad that you're here and are ready to continue on your health journey!
Here's your two-step checklist in order to continue seeing me as your doctor.
1 - Email me at KellyBakerND@gmail.com with 4 pieces of information:
the patient's full name
patient's date of birth
patient's phone number
email address with which you'd like your Charm account associated. This email address can be the same one you used at Healthwise.
2 - Once you receive your confirmation set-up email, electronically fill out and sign consent forms through Charm - These will be available to you in the portal (under Questionnaires) once I've set up your account.
It's that easy! Once you're set up, you'll be able to schedule appointments online through the Charm portal or through the website at www.kellybakerND.com (add whatever the link for the scheduling is). You're also welcome to call the virtual office staff at 206-614-0146 to help schedule an appointment at your earliest convenience. If you have particular quick questions about an existing treatment plan, you'll be able to message me directly through the Charm portal. You can even pay outstanding bills online through the portal, but do keep in mind that outstanding bills from Healthwise need to be settled through Healthwise.
If I could bill your insurance at Healthwise, I can bill your insurance here.
As a reminder I am considered in-network with the following insurance plans: Premera, Regence, Lifewise, Kaiser PPO, First Choice.
I will continue to offer telemedicine appointments! This has been extremely convenient for many of you, as much of the work that we do together can be done very effectively on telemedicine without the time spent driving and parking. For those of you who are fully COVID-19 vaccinated and interested in having in-person appointments as the pandemic allows, my office is located in Edmonds. Please discuss with me on an individual basis if you're interested in in-person appointments.
I can request to have access to all of your chart notes and records from your appointments at Healthwise Integrative Medicine. One of the consent forms included in the new Questionnaires will be a medical records authorization release for your previous Healthwise records; once signed, we can expect your records to arrive within 6 weeks, usually sooner.
